STANFORD, Calif. Reconnecting severed blood vessels is mostly done the same way today with sutures as it was 100 years ago, when the French surgeon Alexis Carrel won a Nobel Prize for advancing the technique. Now, a team of researchers at the Stanford University School of Medicine has developed a sutureless method that appears to be a faster, safer and easier alternative.
In animal studies, a team led by Stanford microsurgeon Geoffrey Gurtner, MD, used a poloxamer gel and bioadhesive rather than a needle and thread to join together blood vessels, a procedure called vascular anastomosis. Results of the research will be published online Aug. 28 in Nature Medicine. Lead authors of the study were Stanford postdoctoral scholar Edward Chang, MD, and surgery resident Michael Galvez, MD.
The big drawback of sutures is that they are difficult to use on blood vessels less than 1 millimeter wide. Gurtner began thinking about alternatives to sutures about a decade ago. "Back in 2002, I was chief of microsurgery at Bellevue in New York City, and we had an infant 10 to 12 months old who had a finger amputated by the spinning wheel of an indoor exercise bike," said Gurtner, senior author of the study and professor of surgery. "We struggled with reattaching the digit because the blood vessels were so small maybe half a millimeter. The surgery took more than five hours, and at the end we were only able to get in three sutures.
"Everything turned out OK in that case," he continued. "But what struck me was how the whole paradigm of sewing with a needle and thread kind of falls apart at that level of smallness."
Sutures are troublesome in other ways, too. They can lead to complications, such as intimal hyperplasia, in which cells respond to the trauma of the needle and thread by proliferating on the inside wall of the blood vessel, causing it to narrow at that point. This increases the risk of a blood clot getting stuck and obstructing blood flow. In addition, sutures may trigger an immune response, leading to inflamed tissue that also increases the risk of a blockage.
The new method could sidestep these problems. "Ultimately, this has the potential to improve patient care by decreasing amputations, strokes and heart attacks while reducing health-care costs," the authors write in the study.
Earlier in his career, as Gurtner contemplated a better way of joining together blood vessels, he considered whether ice could be used to fill the lumen, the inner space of the blood vessel, to keep both ends open to their full diameter long enough to glue them together. Not feasible, he concluded. "Water turns to ice quite slowly and you would have to drop the temperature of the surgical site a lot from 98.6 degrees to 32 degrees Fahrenheit," he said.
Shortly after arriving at Stanford in 2005, Gurtner approached fellow faculty member Gerald Fuller, PhD, professor of chemical engineering and the Fletcher Jones II Professor in the School of Engineering, about whether he knew of a substance that could be turned easily from a liquid to a solid and back to a liquid again, and that would also be safe to use in vascular surgery. Fuller immediately suggested a Food and Drug Administration-approved thermoreversible poloxamer called Poloxamer 407. It is constructed of polymer blocks whose properties can be reversed by heating.
Fuller teamed up with Jayakumar Rajadas, PhD, director of the Stanford Biomaterials and Advanced Drug Delivery Laboratory, to modify the poloxamer so that it would become solid and elastic when heated above body temperature but dissolve harmlessly into the bloodstream when cooled. The poloxamer then was used to distend both openings of a severed blood vessel, allowing researchers to glue them together precisely.
The researchers used a simple halogen lamp to heat the gel. In tests on animals, the technique was found to be five times faster than the traditional hand-sewn method, according to the study. It also resulted in considerably less inflammation and scarring after two years. The method even worked on extremely slim blood vessels those only 0.2 mm wide which would have been too tiny and delicate for sutures. "That's where it really shines," Gurtner said.
Dermabond, a surgical sealant, was used to attach the ends of the blood vessels together.
Poloxamers have been used before as a vehicle for delivering drugs, including chemotherapeutics, vaccines and anti-viral therapies. Researchers have used Poloxamer 407 to occlude blood vessels in experimental animals for the purpose of evaluating the gel's safety and efficacy in so-called "beating heart surgery," in which certain vessels need to be temporarily blocked to improve visibility for the surgeons performing a coronary artery bypass.
Although other sutureless methods have been developed, they generally have not produced better outcomes, the authors said. "Often, the use of microclips, staples or magnets is itself traumatic to blood vessels leading to failure rates comparable to or higher than sutured anastomoses," they wrote.
"This is a novel approach to anastomosis that could play a valuable role in microvascular surgery," said Frank Sellke, MD, chief of cardiothoracic surgery at Brown University Medical Center and associate editor of the Journal of Thoracic and Cardiovascular Surgery, who was not involved in the study. "But it really needs to show that it holds up in clinical trials."
The authors say further testing on large animals is needed before human trials can begin, but they note that all of the components used in the technique are already approved by the FDA. "This technology has the potential to progress rapidly from the 'bench to bedside,'" they write.
Gurtner said he believes the new technique could satisfy a huge unmet need and prove especially useful in minimally invasive surgeries, in which manipulating sutures takes on a whole new level of difficulty.
Michael Longaker, MD, the Deane P. and Louise Mitchell Professor in the School of Medicine and a co-author of the study, called the technique a "potential game-changer."
"When you're bringing together hollow tubes, whether they're large structures, like the colon or the aorta, or a small structure, like a vein in the finger of a child, you're always worried about lining them up directly and effectively sealing them," Longaker said. "The technique that Dr. Gurtner has pioneered could allow surgeons to perform anastomosis more quickly and with improved precision."
He continued: "Coming up with this solution was the result of the classic Stanford model of bringing together researchers from a variety of disciplines."

SNOQUALMIE, Wash. ? Fred Couples has told Tiger Woods that he decided to add him to the 12-member Presidents Cup team.
Couples took the drama out of the debate over whether Woods would be on the team, saying Thursday it's already a done deal.
"I've told him that he's going to be on the team," Couples said just before a practice round preparing for the Boeing Classic at TPC Snoqualmie Ridge. "There is no reason for me to wait till Sept. 26 to pick Tiger. He's the best player in the world forever."
Ten players automatically qualify for the team based on points accumulated over the past two years. Couples is then given two captain's picks to round out his squad, which will compete against a team of non-European International players at Royal Melbourne in Australia on Nov. 14-20.
Woods, who is 28th on the Presidents Cup list, had a long summer layoff because of soreness in his left knee and left Achilles' tendon. He played in two recent tournaments, finishing 37th in the Bridgestone Invitational in early August then missing the cut the following week at the PGA Championship.
Woods wrote on his website earlier this week that he had conversations with Couples about playing on the team and that "of course, I want to be on the team" that competes in Australia.
"Is he playing well right now? No. (But) he almost won (The Masters) four months ago so you don't do that by playing poor golf," Couples said. "In my opinion, when you're the best player in the world for 12 straight years and you're not on a team, there's something wrong.
"Everyone else can have their opinion, but as far as I know Jay Haas and myself are the captains and we want him on this team."
Haas is Couples' assistant captain for these matches.
Couples, initially unaware the Woods had not qualified for this week's FedEx Cup, told him by text message to find another tournament to play. Woods' next scheduled appearance is not until the Australian Open, the week before the Cup, although he said he might add a PGA Tour event to his schedule.
"He has responded to that. I'm pretty sure you'll see him one more time," Couples said. "That's enough for me. I know the last thing he wants to do is embarrass himself. I think he'll be ready to play. Does that mean he's going to go out and win every point? No, I don't expect that. But I expect him to be sharp and ready to play. I would be shocked if he's not."
Couples has requested that all his team members play in the Australian Open to sharpen their games before the Presidents Cup. The Australian Open is Nov. 10-13 at The Lakes near Sydney.
One of the players on the edge of the Presidents Cup standings is Bill Haas, son of Jay Haas. He is ranked 10th with a month to go before the Sept. 26 selection date. Jim Furyk and Rickie Fowler are 11th and 12th.
"I don't know how you can criticize someone for choosing Tiger Woods. If he goes there and doesn't play well I would be shocked. I have a pairing for him. He deserves to be on the team," Couples said. "There are guys who are 11, 12, 13 who are having phenomenal years. But you know they're picks. I could take the easy way out and just pick 1 through 12. I don't think if that works either. I think that's the easy way out."
LOS ANGELES ? Actress Jaime Pressly has been sentenced to three years of informal probation after pleading no contest to drunken driving. Santa Monica Deputy City Attorney Melanie Skehar says Pressly's attorney entered the plea Thursday for the former star of "My Name is Earl."
Pressly was also ordered to attend a six-month alcohol education class. Skehar says the Department of Motor Vehicles is also requiring an ignition locking device be installed on her car.
Pressly was arrested Jan. 5 after police say she was stopped for a traffic violation and charged with driving while having a blood alcohol content of more than .20. The legal limit is .08.
Her attorney, Richard Hutton, did not immediately return a phone message seeking comment.

The latest version of that midsize model has again generated good reviews and buyer enthusiasm, and so has a more stylish remake of the SRX crossover. But two strong products can?t carry a brand in today?s luxury market, analysts caution, especially since Cadillac?s lead rivals are undergoing massive product proliferation.
Consider Mercedes. Two decades ago the automaker debated whether to add the small C-Class to its line-up, which then consisted of the mid-luxury E-Class, the big S-Class and the SL roadster. Today, the automaker?s line-up includes the even smaller A- and B-Class offerings, the GLK, the G- and M-Class crossovers, the V-Class van and a long list of sedans and coupes.
?I think they?ve acknowledged they?ve fallen behind,? said Dave Sullivan, an automotive analyst at AutoPacific, referring to Cadillac. ?They?re no longer the ?standard of the world,? as they?ve long liked to proclaim.?
It?s not that the Detroit brand doesn?t want to regain that image, and new products will be critical to reclaiming the high ground. Alongside the second-generation CTS sedan, Cadillac has added a striking new coupe version, a station wagon and super-high-performance V-Series versions.

Other new vehicles are in the works. Last week, GM officials confirmed they had reversed a previous decision and will now bring the well-reviewed Converj concept to market. SALT LAKE CITY (AP) ? A bio-art project to create bulletproof skin has given a Utah State researcher even more hope his genetically engineered spider silk can be used to help surgeons heal large wounds and create artificial tendons and ligaments.
Researcher Randy Lewis and his collaborators gained worldwide attention recently when they found a commercially viable way to manufacture silk fibers using goats and silkworms that had spider genes inserted into their makeup.
Spider silk is one of the strongest fibers known and five times stronger than steel. Lewis' fibers are not that strong but much stronger than silk spun by ordinary worms.
With Lewis' help, Dutch artist Jalila Essaidi conducted an experiment weaving a lattice of human skin cells and silk that was capable of stopping bullets fired at reduced speeds.
"Randy and I were moved by the same drive I think, curiosity about the outcome of the project," Essaidi said in an email interview. "Both the artist and scientist are inherently curious beings."
Lewis thought the project was a bit off the wall at first, Essaidi acknowledged.
"But in the end, what curious person can say no to a project like this?" she said.
Essaidi, who used a European genetics-in-art grant to fund her project at the Designers & Artists 4 Genomics Awards, initially wanted to use Lewis' spider silk from goats to capitalize on the "grotesque factor" of the mammal-spider combination.
But Lewis didn't yet have enough of the spider goat silk to send hundreds of yards to Essaidi. So he sent her spools of silk from silkworms he had genetically engineered in a fashion similar to the goats.
Essaidi initially intended to fire .22 caliber bullets at the "skin" stretched in a frame. But she decided to place the "skin" on a special gelatin block used at the Netherlands Forensic Institute.
Using a high-speed camera, she showed a bullet fired at a reduced speed piercing the skin woven with an ordinary worm's silk But when tested with Lewis' genetically engineered worm's silk grafted between the epidermis and dermis, the skin didn't break. Neither was able to repel a bullet fired at normal speed from a .22 caliber rifle.
"We were more than a little surprised that the final skin kept the bullet from going in there," Lewis said of the tests at reduced speed. "It still ended up 2 inches into the torso, so it would not have saved your life. But without a doubt the most exciting part for us is the fact that they were able to recreate the skin on top of our fibers. It's something we haven't done. Nobody has worked in that area."
Essaidi was intrigued by the concept of spider silk as armor, and wanted to show that safety in its broadest sense is a relative concept, hence bulletproof.
"If human skin would be able to produce this thread, would we be protected from bullets?" she wondered on her blog. "I want to explore the social, political, ethical and cultural issues surrounding safety in a world with access to new biotechnologies."
She said it is legend that Achilles was invulnerable in all of his body except for his heel.
"Will we in the near future due to biotechnology no longer need to descend from a godly bloodline in order to have traits like invulnerability?" she asked.
Lewis downplayed the potential bulletproof applications of his research.
"I certainly would not discount that, but I don't see that as a tremendous application at the moment," he said.
He said bulletproof vests already exist. But being able to grow cells and use the material to replace large amounts of human skin could be significant for surgeons trying to cover large wounds, or treat people with severe burns.
He said the material's strength and elasticity would enable doctors to cover large areas without worrying about it ripping out ? a big advantage over small skin grafts.
Lewis couldn't give a time frame for such a use because it would require FDA approval. But he hoped to do some animal testing within two years, and noted spider silk already has proven very compatible with the human body.
The next step is to generate more material to test what cells will grow on it ? made easier with the "transgenic" silk worms and milk from goat spiders.
The real stuff is still the holy grail for fibers and textiles but not the easiest to come by as evidenced by an 11-by-4 foot tapestry unveiled two years ago at the New York Museum of Natural History that took millions of spiders to complete.
"We know some skin cells will grow (on our fibers), but can we get cells that make ligaments and tendons grow," Lewis said.
He said it may be easier to use the genetically engineered silk to make materials better than actual ligaments or tendons.
Essaidi, meanwhile, said she has plenty of wild ideas but wants to transplant the bulletproof skin.
She said Geert Verbeke, director of Verbeke Foundation in Belgium, the biggest Eco/BioArt museum, wants to wear the skin "as an ode to BioArt."
Back at Utah State's bio-manufacturing facility in Logan, Utah, Lewis just started breeding for the next round of milking in January. He has about three dozen of the genetically engineered goats. He extracts proteins from the special milk then spins them in a way that replicates the spider's method, resulting in a strong, light-weight fiber.
"Nothing is as strong as the natural fiber, yet," Lewis said of spider silk. "But we are working on solving that problem."

NEW YORK (Reuters) ? A Pakistani immigrant pleaded guilty on Thursday in federal court to running an illegal money-transfer business that provided $7,000 to the man who tried to bomb New York's Times Square.
Mohammad Younis, 45, was charged in September, five months after Faisal Shahzad was arrested for parking a crude car bomb in the crowded Times Square on May 1, 2010. A bomb squad ultimately defused the device.
Federal prosecutors in Manhattan said in the indictment that Younis met with Shahzad on April 10 last year and gave him the money sent by people in Pakistan as part of an informal "hawala" money transaction common in Islamic societies.
On Thursday, Younis pleaded guilty in Manhattan federal court to one count of conducting an unlicensed money transfer business. But Shahzad's name was not mentioned in court.
Younis told U.S. District Judge John Keenan that he did not know who he gave the money to or what the money was intended for. Prosecutors have not charged Younis with participating in or having any knowledge of Shahzad's planned attack.
"Specifically, I transferred $5,000 to one individual and $7,000 to the other individual," Younis said in court.
Federal prosecutors have not said who the recipient of the $5,000 payment was.
Younis, a Pakistani citizen, is slated to be sentenced on November 30 and will likely face deportation after sentence. His recommended sentence spelled out in a plea agreement with prosecutors is for up to six months in prison.
Shahzad was sentenced to life in prison in October over the failed bombing.

BERLIN, Aug. 20, 2011 (Reuters) ? Germany on Saturday rebuffed renewed calls that euro zone countries should issue joint euro-denominated bonds and have a joint finance minister, arguing that would only be possible if fiscal policy were collective already.
"As long as we don't collectivise financial policy we also cannot have a uniform interest rate level. The different rate levels are the incentive to run a solid economy or the punishment if you are not running it properly," Finance Minister Wolfgang Schaeuble, speaking at his ministry's open day.
"So the question is, how do we manage to promote political integration step by step. We cannot collectivise interest rates," Schaeuble said, referring to proposals that the euro currency bloc should issue common euro bonds.
Germany has led resistance to calls that the euro currency bloc should issue common euro bonds and expand its bailout fund to calm repeated market selloffs of government bonds and bank shares of vulnerable debtor countries.
Der Spiegel magazine reported finance ministry calculations that showed issuing joint euro bonds would cost Germany billions of euros each year.
However, Martin Blessing, Chief Executive of Germany's second-largest lender Commerzbank said a European finance minister with sway over member states' taxes and budget was needed to lead the euro zone out of its debt crisis.
Berlin is also facing criticism over its own proposals to solve the euro zone crisis, which include a financial transaction tax that Chancellor Angela Merkel and French President Nicolas Sarkozy said this week they would propose to other euro zone members.
Schaeuble is to meet his French counterpart Francois Baroin in Paris on Tuesday to discuss the health of Europe's finances including remedies such as the tax.
Andreas Schmitz, the head of the association of German banks (BdB), whose clients would be directly affected by the tax, said the tax would be inefficient and would not prevent financial crises as professional traders did not care where they traded and would simply avoid Europe.
"The big tax income will fail to appear," he told Bild am Sonntag newspaper. Proponents of the tax expect it could raise 30 to 50 billion euros a year.

NEW YORK (AP) ? Stores are trying everything they can think of to disguise the fact that you're going to pay more for clothes this fall.
Some are using less fabric and calling it the new look. Others are adding cheap stitching and trumpeting it as a redesign. And the buttons on that blouse? Chances are you're not going to think it's worth paying several dollars more for the shirt just to have them.
Retailers are raising prices on merchandise an average of 10 percent across-the-board this fall in an effort to offset their rising costs for materials and labor. But merchants are worried that cash-strapped customers who are weighed down by economic woes will balk at price hikes. So, retailers are trying to raise prices without tipping off unsuspecting customers.
"Let the consumer trickery begin," said Brian Sozzi, Wall Street Strategies retail analyst
Retailers have long tried to mask price hikes ? for instance, jacking them up more than needed so that they can offer a "sale" on the higher price. But the new strategies come as merchants' production and labor costs are expected to rise 10 percent to 20 percent in the second half of the year after having remained low during most of the past two decades. Costs can quickly add up: Raw materials account for 25 percent to 50 percent of the cost of producing a garment, while labor ranges from 20 percent to 40 percent, analysts estimate.
Stores already have passed along their rising costs to customers by raising prices on select items. The core Consumer Price Index, which includes spending on everything except food and energy, rose 0.2 percent in July, the Labor Department said Thursday. But now that production costs are going up even higher, merchants are increasing prices on a broader range of merchandise. Because of their concern that shoppers will retreat, though, retailers are treading the line between style, quality and price.
Some merchants are making inexpensive tweaks __ additional stitching, fake button holes, fancy tags __ to justify price increases. Those embellishments can add pennies to $1 to the cost of a garment, but retailers can charge $10 more for them, said Marshal Cohen, chief industry analyst with market research firm The NPD Group.
"We're not seeing deflation or inflation; we're seeing con-flation," he said. "Stores are making consumers believe their getting more for their money."
After the price of the fabric for its girl's corduroy pants almost doubled, catalog retailer Lands' End, based in Dodgeville, Wis., raised the price of the pants by $7 to $34.50. The company, a unit of Sears Holdings Corp., also added buttons and stitching on the pockets to dress them up.
"Consumers are going to notice the price differences," said Michele Casper, a Lands' End spokeswoman. "But they are also going to get a lot of added benefits so they know they're not getting short-changed."
Others are taking away things, but marketing it to customers as the latest trend.
Spencer Elmen, owner of Cupid's Lingerie, which operates five stores in Arkansas, said he is seeing more items in his store that are even skimpier than usual, from underwear to mini dresses. He says that's because designers are finding clever ways to conceal the fact that they're clothes have less fabric.
Elmen said $39.99 teddies, which are $5 more than that they were last year, feature a studded heart that gathers up the material to disguise the fact that less fabric is being used. He also noted that the corsets with fishnet patterns are priced about 5 percent more at about $49, even though they also have less material.
"They're just being more creative with less fabric," Elmen said.
Teen retailer Abercrombie & Fitch is advertising "Redesigned 2012" jean collection in its stores and on its website, touting that the jeans are "softer, with the perfect amount of stretch." They're also mostly priced between $78 and $88, about $10 more than last year, according to Jennifer Black, who heads up research firm Jennifer Black & Associates.
Sozzi, the Wall Street Strategies retail analyst, examined the jeans and believes they are "thinner" and of "cheaper quality." That extra stretch, he says, simply could mean the retailer is saving costs by using less denim.
Eric Cerny, an Abercrombie & Fitch spokesman, declined to comment. But Cerny reiterated what executives told investors in recent months: the bulk of increases on items will start to happen in September and the chain will not sacrifice quality to achieve cost reductions.
Bill Melnick, director of strategic planning at SAI Marketing, which studies consumer behavior at major consumer brands, said most shoppers may not notice retailers' tactics to disguise prices. But he says shoppers won't buy if they can't afford it.
"Shoppers are being pragmatic," he says, nothing that they think "'If it fits into my budget, then it's a sale.'"
Rhonda Sayen, a Stephens City, Va., resident, said she checked out prices on new fall items and noticed jeans that were about $40 a year ago are now closer to $60. She also said she's spotted lower quality T-shirts at some of the stores.
"I know prices have changed," said Sayen, who added that she and her husband are sticking to a $400 budget for clothing and supplies for her four children ages 3 to 18. "You ain't fooling me."
